    How to use the template

    1. Fill in one form for each accident, as soon as possible after it happens, once the person has received any first aid they need.
    2. The injured person can complete the form, or someone can do it on their behalf, such as a first aider, colleague or manager.
    3. Stick to facts: what happened, where, when and the injury. Do not record opinions about blame.
    4. The details in the first five sections cover what the Social Security (Claims and Payments) Regulations 1979 require for an accident book entry.
    5. Some accidents must be reported to the HSE under RIDDOR. Check with the free RIDDOR checker at useaccidentbook.co.uk/riddor-checker.
    6. The form contains personal and health data. Store completed forms securely and only share them with people who need to see them, in line with UK GDPR.

    Frequently asked questions

What should an accident report form include?+
The injured person’s name, address and occupation; the date, time and exact place of the accident; what happened and what caused the injury; the nature of the injury; first aid given; witnesses; and, if someone else fills it in, their details. A manager should then record whether it is RIDDOR reportable and the actions taken.
Is an accident report form the same as an accident book?+
They record the same information. An accident book is a set of records kept together, often with tear-out pages, while an accident report form records one accident at a time. Either can meet the legal requirement, as long as the required details are recorded and stored securely.
Who should fill in an accident report form?+
The injured person, or someone on their behalf, such as a first aider, colleague or manager. It should be completed as soon as possible after the accident, once the person has had any first aid they need.
Do I need to send the accident report form to the HSE?+
No. Accident report forms are internal records. Only RIDDOR-reportable incidents, such as specified injuries and over-7-day injuries, must be reported to the HSE, using the HSE’s online forms.
How long should accident report forms be kept?+
At least 3 years, as for accident book records and RIDDOR records. Many employers keep them longer because of the time limits for personal injury claims.
