    How to use an accident book template

    1. Use one page for each accident. Keeping records on separate pages protects people's personal data, as required by UK GDPR.
    2. Fill in the record as soon as possible after the accident. The injured person, or someone acting for them, can complete it.
    3. Once complete, remove the page and store it securely (for example in a locked cabinet) where only authorised people can see it.
    4. Keep records for at least three years from the date of entry. Records about children are often kept until they turn 21.
    5. Check whether the accident is reportable under RIDDOR. Deaths and specified injuries must be reported without delay, and over-7-day injuries within 15 days.
    6. Record near misses too. They help you spot hazards before someone is hurt.

    The problem with paper accident books

    A template is a good start, but paper records have limits. Books get lost or damaged, pages are filled in without the details you need, and nobody spots a RIDDOR-reportable injury until the deadline has passed.     Frequently asked questions

    Do I have to use the HSE accident book (BI 510)?+
    No. The BI 510 is the HSE’s published accident book, but the law does not require a particular book or form. Any record is acceptable as long as it captures the required details, protects personal data and is kept for at least three years.
    Is this accident book template legally compliant?+
    The template captures the details required by the Social Security (Claims and Payments) Regulations 1979: the injured person’s name, address and occupation, the date, time and place of the accident, the cause and nature of the injury, and the details of the person making the entry if it is not the injured person. It also includes RIDDOR and follow-up fields that inspectors expect to see.
    How do I keep a paper accident book GDPR compliant?+
    Use one page per accident, remove completed pages and store them somewhere secure, such as a locked cabinet, so other people cannot read earlier entries. Only authorised people should have access, and records should be destroyed securely when your retention period ends.
    Who needs an accident book?+
    Employers with 10 or more employees at any one time, and those who own or occupy a mine, quarry or factory, must keep an accident book under the Social Security (Claims and Payments) Regulations 1979. Most other businesses keep one anyway, because it is the simplest way to meet their wider health and safety and RIDDOR duties.
    Can I keep my accident book online instead?+
    Yes. A digital accident book is acceptable as long as it records the same information securely. It also stops pages being lost, makes records easier to search, and can flag incidents that may be RIDDOR reportable.
